GENERAL QUESTIONS


+ What can I expect at my initial physical therapy evaluation?

Dr. Erin Kottich, DPT, will meet with you in-person or virtually to discuss your symptoms, health history, goals and treatment options. A thorough examination will be completed to check mobility, strength, balance, posture and muscle imbalances. The session will also include manual therapy, exercise prescription and/or patient education.

+ What should I wear to my physical therapy/pilates sessions?

We recommend wearing non-restrictive, comfortable clothing, like you would wear during a workout.

+ What are the accepted forms of payment for services?

Cash or check are the preferred payment forms. Payment is required at the time of service.

+ Do I need a referral from a doctor before being treated at InspireLife?

Nebraska is a direct access state, and as such, a physician’s referral is not required to make an appointment or receive treatment. However, if you would like reimbursement for physical therapy services, we do recommend calling your insurance company to see if a referral is needed.

+ What are InspireLife’s physical therapy session rates?

Initial Evaluation + Treatment: $150
30-Minute Treatment Session: $60
60-Minute Treatment Session: $120

+ When is my payment due?

Payment is required at the time of service. InspireLife accepts cash and check.

+ Does InspireLife accept insurance?

No, InspireLife uses a self-pay model that allows for a unique, patient-focused experience. We have redefined the rehabilitation process by focusing on our patients’ specific needs. This model allows us to focus on the whole body, overall wellness, patient education and prevention without restrictions that come from an insurance-based model. We provide a high level of one-on-one care with an emphasis on manual techniques, patient education and exercise prescription.

+ Can non-Medicare beneficiaries use insurance?

Inspire Life is an out-of-network physical therapy provider and payment is due at the time of service. Upon request, we can provide a detailed invoice that may be turned into your insurance provider. If you would like to submit physical therapy services to your health insurance provider, it is important that you contact your provider to confirm coverage and any information they may need to file a claim prior to scheduling your initial appointment. It is also important to determine if your insurance company requires a doctor's referral in order to be seen by a physical therapist. Although it is not a requirement in the state of Nebraska, some insurance companies require a physician referral in order to reimburse you for physical therapy services.

+ Considerations for Medicare beneficiaries:

Due to current healthcare regulations, a cash-based physical therapy provider is allowed to see a Medicare beneficiary only if that beneficiary insists on not involving Medicare.

At InspireLife, there are two scenarios in which you may be seen due to these regulations:

  1. For Pilates one-on-one or group sessions, personal training and wellness visits. Because these services are not covered by Medicare and therefore cannot be billed to Medicare, you are free to schedule and pay for these services out-of-pocket without any restrictions
  2. If you are clear and adamant that you do not want to involve Medicare in physical therapy services. If you insist of your own free will that Medicare will not be notified of, billed for or involved in your physical therapy care in any way, you may give authorization for treatment. If you wish to use your Medicare benefits for physical therapy services, we are happy to refer you to a participating Medicare provider.
